Why Am I Putting Weight Around My Middle During Menopause?

One of the most common questions I get asked by women in their 40s and 50s is:
"Jen, why am I suddenly gaining weight around my middle when I haven't really changed anything?"
If this sounds familiar, you're certainly not alone.
Many women find that during perimenopause and menopause, their body shape starts to change. Weight that once settled around the hips and thighs now seems to gather around the waistline, and the frustrating part is that it can happen even when you're eating much the same as you always have.
So what's going on?
It's Not Just About Calories
Whilst calories do matter, menopause-related weight gain is about far more than simply eating too much.
Several changes are happening at the same time:
Declining Oestrogen
As oestrogen levels fluctuate and eventually decline, the body becomes more likely to store fat around the abdomen.
Before menopause, women typically store more fat around the hips and thighs. As oestrogen falls, fat distribution often shifts towards the middle, creating the classic "menopause belly."
This isn't a lack of willpower. It's a hormonal change that many women experience.
Age-Related Muscle Loss
From our 30s onwards, we naturally begin to lose muscle mass unless we're actively working to maintain it.
Muscle is metabolically active tissue, meaning it burns more energy than fat, even at rest.
The less muscle we have, the fewer calories we burn throughout the day.
This is one reason why the strategies that worked in your 20s and 30s often stop working in your 40s and 50s.
Stress and Cortisol
Many women in midlife are juggling careers, ageing parents, grown-up children, financial pressures and poor sleep.
Chronic stress can lead to elevated cortisol levels.
When cortisol remains high for long periods, it can increase cravings, make it harder to regulate blood sugar and encourage fat storage around the abdominal area.
This is one reason why simply eating less and exercising harder isn't always the answer.
Blood Sugar Rollercoasters
Many women unknowingly spend the day on a blood sugar rollercoaster.
A breakfast of cereal or toast, a mid-morning biscuit, a sandwich lunch and an afternoon snack can cause repeated spikes and crashes in blood sugar.
These fluctuations can:
Increase hunger
Trigger cravings
Reduce energy levels
Encourage fat storage
This is why I focus heavily on protein, healthy fats and plenty of vegetables in my nutrition plans.
The Good News
Menopause does not mean you are destined to gain weight forever.
The women who get the best results typically focus on:
Strength training to build and maintain muscle
Prioritising protein at every meal
Managing stress levels
Improving sleep quality
Reducing ultra-processed foods
Following a sustainable plan rather than extreme diets
In fact, many of the women I coach achieve some of their best results after 40 once they start working with their changing hormones instead of fighting against them.
